Symptom: People in the organization are unhappy and demotivated.

Unhappy team members (within the organization) inevitably mean unhappy clients and poor business results. It's not that employees come before clients, or vice versa. Client satisfaction and employee satisfaction are integrally linked.
Aside from an uncaring boss (which is an individual, not an organizational, issue), there are a variety of systemic reasons why people might be unhappy working within an organization.

Symptom: People are chronically overworked; i.e., there is more work expected than time available.

Symptom: People feel disempowered, and unable to control all they need to accomplish their accountabilities.

Symptom: People feel that their performance doesn't matter.

Symptom: People don't perceive a chance to get ahead, i.e., a rewarding career path.

Symptom: People think that all the new, interesting jobs are given to contractors and the organization isn't willing to invest in them.

Symptom: People feel "jerked around" and have lost faith in management due to constant changes in the organization.

Symptom: People feel disconnected from the organization and isolated from their peers.

Symptom: People don't feel that they are contributing to a worthwhile mission.
