Symptom: People have trouble keeping their promises to clients and peers alike.

If the problem is widespread -- that is, if people have trouble keeping commitments to everyone -- then more than teamwork will suffer.


Another (more generic) way to state the problem is as follows:
Symptom: Clients complain that the organization is unresponsive, or that the its products are not delivered in a timely fashion.
