Symptom: People are not producing satisfactory work (responsiveness, quality, cost) in a variety of groups throughout our organization.

Producing work efficiently and effectively means responding quickly to clients' requests with high quality products at a fair cost. Responsiveness, quality, and cost are all tightly related. They are all outcomes of a healthy, productive organization.
Millions of things might cause production problems. The only way to deliver products reliably is to design an organization that ensures delivery everywhere, all of the time.
There are a number of reasons why people might not be well focused on product delivery. It may be that people don't care about results; or perhaps people have incentives that discourage them from some results.
In some cases, people are not able to implement process improvements. Part of the answer is an organization that makes individuals and small groups accountable for products. Another part of the answer is to empower them to run their piece of the business. Another is to give people the resources and feedback they need to produce quality.

Symptom: People don't feel a passion for better serving their customers with quality.

Symptom: People really want to better satisfy their customers, but cannot.
