Symptom: People don't generally try to form clear agreements.
At the start of every project as the team is formed, a project plan should delineate clear deliverables for each team member. These agreements can be considered "contracts" between the prime contractor (project leader) and subcontractors (team members), or between a subcontractor and his or her subcontractor.
Forming project teams in this manner should be a "habit" built into the organization's culture.
