Symptom: People in the organization have reasons not to improve their efficiency.

There may be strong incentives not to change and improve things. For example, in some organizations, when it comes to reducing costs, commanding a larger budget (higher costs) gives one greater status, title, and compensation. These reversed incentives are a rewards system gone awry.
