Symptom: People have not been given the authority they need to manage on their own.

Qualified people will fail -- and necessitate intervention by their boss -- if they don't have the authority they need to accomplish all that's expected of them. Put another way, the boss gets drawn into the details if people must involve the boss whenever any decision making or authority is required.
This is a symptom of the lack of whole jobs -- jobs defined around product lines, where people have all of the authority and accountabilities associated with running a "business within a business." That is, disempowerment may be designed into the organization chart.
