Symptom: There are other groups in the organization that monitor and control the people doing the work.
Audit means inspecting the work of others and perhaps vetoing their decisions. It is a direct form of disempowerment.
With someone else checking on their work, many people feel that they are excused from responsibility for their own quality. "Don't worry about quality," they might say; "the inspector at the end of the assembly line will catch defects."
Another (more generic) way to state the problem is as follows:
