Symptom: Teams don't work well.

Project estimates may be realistic, project management techniques may be effective, and technical skills may be adequate -- but if teams don't work well together, results will be disappointing.
Effective teamwork is based on a variety of specialists contributing their unique talents to a common cause.


Another (more generic) way to state the problem is as follows:
Symptom: While individuals and small groups may be capable, the organization is not good at cross-boundary teamwork.
